Being geared low give it more pick up, not less.
First thing to do is always a compression test.
No point in trying to spend time if the engine has basic problems.
Then you work on timing and mixture.
You want to make sure the spark is as advanced as possible, and that you are getting enough gas.
The includes checking fuel pressure, replacing filter, checking fuel pump, return pressure regulator, etc.
Make sure the sensors all read within range with an ohm meter.
The mass air sensor always needs cleaning because it collects oil/water foam from the PCV.
The throttle position sensor can also be checked.
